Training Course Applications

From the list at the left, select and download the application form for the training you wish to take. Fill in all the information and mail your completed form to the office at the address given on the form. You may also fax your application to the office at 310-391-2168. All personal information is held in strict confidence. Only Heart Touch staff will have access to your application.

Note that payment is due when you submit your application and applications are not considered complete without payment. The fee for each training is listed below. Each application form has an area for credit card information. If you prefer to pay by check, please make it out to The Heart Touch Project and mail it to the office.

• Heart Touch Training in Compassionate Touch

• Advanced Heart Touch Training for End-of-Life Care
  Tuition: $150.00
• Massage for Medically Fragile Infants and Children
  Tuition: $75.00 for Heart Touch volunteers

• Infant Massage Instructor Training
  Tuition: $75.00 for Heart Touch volunteers
  $250.00 for all others
• International Outreach Program
  Non-refundable Deposit: $100.00
  Total Cost: Ranges from $3,800 to $4,500 based on travel and lodging costs
